Accuracy you can audit beats authority you have to take on faith. This page is the permanent public record of every substantive error published in an FSMA Radar issue.

The policy: when we get something wrong, whether a date, an applicability statement, or an interpretation, the correction runs at the top of the next issue and is recorded here permanently, with the issue number, what we said, what was correct, and the primary source. We do not silently edit the record.

Corrections to date

Correction 1 · Issue #7

What we said: Issue #7 (week of August 24–31) attributed the Cyclospora outbreak's case counts, 11,458 illnesses, 495 hospitalizations, and two deaths in Michigan, to "FDA now reports."

What is correct: those surveillance figures come from CDC, not FDA. The two agencies run the investigation jointly: CDC owns the epidemiological case counts, FDA owns the recall and traceback. The FDA page itself notes the case data is "epidemiological information collected by CDC." The operator guidance in the item is unaffected. From issue #8 onward, case counts are attributed to CDC.

Issues #1 through #8 published, covering the weeks of July 16 through September 7, 2026. Issue #4 also carries an editor's note on a coverage gap: the 2026 Cyclospora outbreak should have been covered from issue #1 and was not; the monitoring gap that caused the miss has been closed.
